| Forms and Fees: |
After a place has been offered in the program, it typically takes 3-4 days to process forms and fees. For more information on forms needed plaese check the main Registration page. To download the forms please got to the Form page. If a player trys out on a Monday, Tuesday or Wednesday, we can usually process paperwork in time to play on the weekend. Once we find out your date of birth and the team you are trying for we will send you a fee breakdown. To calculate your age group, use the age group calculator at the PSV Union FC TRYOUT page.
IMPORTANT: Required Before Participation with the team:. (If you are already a registered Union member, no forms are required)
- Must include one (1) copy of the US Club Soccer Release Form found on the following page. Union FC Forms(this item can be scanned and sent electronically)
- Must include Fees. Go to fees section online. To pay for theSpring tuition, go online to the team page and pay using PayPal, or make checks payable to Union FC fees
- Must include copy of birth certificate. (this item can be scanned and sent electronically)
- Must include 2 pictures (1 inch x 1 inch headshots) - Each picture must be a clear, current photo – face forward - head and shoulders – NO POLAROIDS! NO HATS! NO SUNGLASSES! (this item can be scanned and sent electronically)

The fees indicated below do not include uniform fees for new equipment or the following:
- Older teams (96/B & older) competing in US Club Regional's (June 19-22) participate through June 22nd.
- An additional fee (approx $250) for the US Club Regional's in Modesto for 93G, 94B, 94G, 96G, 96B will apply from June 12- June 22. Similar to 2008 and 2009.
- End of season/pre-regional camp will occur Monday June 14- Friday June 18th from 9 am- 12 midday.
- Additional fees do not apply for tournaments and additional team sessions.
Teams practicing 3 days per week + 1 weekend game:
(4 sessions per week x 15 weeks = 60 sessions) $1,108.00
- Coaching 3 practices/week and 1 weekend game = 60 sessions ($960)
- Club Fees ($60)
- League Fees ($23) - $350 based on 15 players
- Referee Fees ($65) - based on 10 games @ $100 per game/15 players

- League Fees ($15) - $200 based on 13 players
- Referee Fees ($45) - based on 10 games x $60 per game/13 players

Ordering Uniforms: |
After placing uniform orders, it typically takes 1 week to 10 days to receive your uniform. Be sure to ask for your favorite numbers. Note that most are already taken if you are joining an existing team. You will be sent uniform information upon receiving confirmation of participation. |

| How to Make Contact and Meet with Admin and Coaches |
We can only meet with you if we have a prior arrangement to do so. Please email us and set up a time to meet and discuss. Before practice is not usually possible. Most questions can be addressed via email. admin.
